The LASIK Procedure
Overview The actual procedure takes only 15 minutes to do both eyes. LASIK is painless and easier than having your teeth cleaned. A mild sedative (like Valium) and anesthetic eye drops are all that are used, to relax you and make you completely comfortable. A small wire spring keeps you from blinking and since your eye is numb you won’t realize your eye is being held open. A sophisticated infrared pupil tracker insures that all eye movements will be safely followed and the laser will precisely etch the intended area.
Patients can usually see quite well immediately after the surgery and frequently resume normal activities within 24 hours, allowing them a quick return to work usually the next day. Over 90% of patients drive themselves back to the return appointment the following day. Minor fluctuations and dryness usually resolve after 1 to 2 weeks.
Before You will have one appointment prior to your LASIK Surgery. During this appointment we will determine your prescription, perform a topography, measure your pupil size and corneal thickness, perform a retina exam and give you a dilated refraction.
The day before the surgery the patient should avoid using all creams, lotions and perfumes. The eyelids should be cleaned using the Ocusoft Lid scrubs provided to you. Also, the patient should arrange for transportation to and from the Laser Eye Institute, you must not drive on the day of your surgery.
The Surgery
The patient arrives about thirty minutes prior to the procedure and is given a mild sedative to reduce anxiety. Eye drops are applied to numb the eye. Next, a small metal ring called a lid speculum is applied to prevent blinking or closing of the eye. The LASIK Surgeon uses an instrument known as a microkertome to create a thin flap on the outer layer of the cornea. This flap is then folded back exposing the corneal tissues to the computerized laser.
The patient is asked to stare at a blinking green light. The laser then removes tiny amounts of the tissue to change the curvature of the cornea thereby, correcting the refractive error. Finally, the flap is replaced without stitches where it can heal naturally. The patient is sent home to sleep for several hours.
After Surgery For the first few hours the vision will be foggy because of the swelling on the cornea, similar to looking through a frosted window or through water. Depending on the amount of treatment performed, the vision is normally much clearer the following morning after the patient has slept all night. Patients should be prepared for blurred vision but normally not as blurred as before surgery without their glasses. Most patients return to work the very next day. LASIK - Post Operation Instructions
As with any outpatient surgical procedure, we rely on our patients to be active participants in managing recovery. Because you or your family member will be providing your care, it is very important to read the information, ask questions, and follow instructions after discharge.
Use the following eye drops: (one drop each)
Antibiotic - 4 times daily for 5 days then discontinue Eye Lubricant - as needed for lubrication for 4-6 weeks. Wait about 1 minute between drops
These drops will be given to you after your LASIK surgery.
After about 30 minutes, the anesthetic drops used in the laser procedure will begin to wear off and some discomfort may begin. It is normal to have the following symptoms: - Watery eyes and nose
- Light sensitivity
- Redness and swelling of the eye
- A sandy feeling (like your contacts dried out)
- Blurry vision
It is rarely necessary to take medication for these symptoms, but should it be needed, Tylenol, Advil or any non-aspirin analgesic can be taken.
Wrap-around sunglasses are helpful in soothing discomfort and shielding your eyes in bright lights.
You will be seen for a post-op visit a week after surgery. Post-operative Do's and Don't's You may shower and take a tub bath, but avoid getting anything in the operated eye. Rest and sleep are your best friends the first day or two.
Wear your eye shield for the first 2 hours and then only while you sleep for the next 3-4 days. Frequently Asked Questions
Is the LASIK procedure painful? No. Several anesthetic drops are given prior to the procedure to eliminate pain and discomfort
What about after surgery? With LASIK, there is virtually no pain. There is a foreign body sensation like having a tiny spec of something in your eye or a dried contact lens for twelve to twenty-four hours. With PRK there can be slight discomfort lasting for several days. A bandage soft contact lens is applied to reduce discomfort and an analgesic eye drop is given, if needed, for pain. Pain medications are typically prescribed for this procedure.
How long will it take to recover from LASIK? With LASIK, it takes most patients less than twenty-four hours to gain functional visual recovery and return to work. Nearly all LASIK patients report being able to see better immediately after surgery.
What are Risks and Possible Complications?
Some people experience poor night vision after LASIK surgery may result in under correction or overcorrection, which can often be improved with a second surgery. More rare and serious complications include a dislocated flap, epithelial ingrowth and inflammation underneath flap. Most complications can be managed without any loss of vision. Permanent vision loss is very rare.
Could I do something during procedure to ruin results? Blinking of eye is prevented by an eyelid speculum that holds eye open. During procedure, patient's responsibility is to be very still during creation of flap with microkeratome and to focus on a red blinking light in microscope for laser part of treatment. Throughout the procedure the physician is observing proper alignment using the microscope. In case of slightest misalignment, procedure is paused and alignment is corrected prior to procedure continuing.
If the patient were to move suddenly during creation of the corneal flap, an irregular flap or tissue cap could be created that would result in the discontinuance of the laser portion of the procedure. In this case, the eye would be given time to heal over the course of a few months and then the procedure could be attempted again.
Does insurance cover LASIK procedure? In most cases, insurance does not cover LASIK procedure. You should expect to pay for your procedure in full at time of surgery.
When can I see clearly?
Your vision is usually dramatically improved within the first twenty-tour hours. It is normal for vision to fluctuate from day to day, especially for the first couple of weeks. How quickly your vision clears depends, sometimes on the severity of your original prescription. Normally it takes six to eight weeks for your vision to stabilize completely.
Avoid all water sports and activities (hot tubs, whirlpools, swimming, etc.) for 2 weeks post-operatively. Avoid scuba diving for 6 weeks.
